Description

  • Collaborates with case managers, payers, referral sources, home health agencies and Apria intake staff to facilitate patient discharge and coordination of care.
  • Obtains supportive documentation for clinical and financial patient care management.
  • Provides patient/caregiver initial education for nutrition therapy and technology.
  • Assesses method of administration based on orders, diagnosis and route of feeding.
  • Communicates clinical follow-up information to case managers after patient discharge
  • Identifies on-site educational opportunities with hospital staff. 
  • Provides continuing education programs as needed to on-site hospital and payer case managers. 
  • Introduces formulary changes and advances in nutrition support.
  • Identifies and develops strategic relationships within the institution that will enhance patient care.
  • Facilitates communication amongst all members of the patient care team.
  • Identifies opportunities for improvement in patient care.
  • Participates in the institution's quality assurance/performance improvement initiatives as requested.
  • Performs nutrition assessments and provides recommendations as needed.
  • Performs other related duties as directed by supervisor.

Qualifications

  • At least one year related experience is preferred, but not required.
  • A licensed registered dietitian in appropriate state of practice is preferred.

Founded in 1978, VNSNY'S Long Term Home Health Care Program (LTHHCP) is the oldest program of its kind in the New York metropolitan area, as well as the largest in the US. The services we provide enable patients with chronic illness or disabilities to live at home instead of in a nursing home facility. These services include skilled nursing care, medical social work, nutrition counseling, rehabilitation services, and home health aide services, and are provided to patients throughout Brooklyn, the Bronx, Manhattan, and Queens.

Responsibilities include performing comprehensive nutrition assessments of assigned patients, developing care plans, and providing medical nutrition therapy, clinical guidance, nutrition education and counseling to meet normal and therapeutic needs.  You will document patient care information such as evaluation data, treatments, communications on patient progress, patient responses to interventions and desired goals.  Participate in agency in-service education programs, QI activities, interdisciplinary team meetings and program marketing activities.

Requires:  Registered Dietitian license approved by the Commission on Dietetic Education of the American Dietetic Association and current Certification to practice in NY State as a Certified Dietitian-Nutritionist required.  Minimum one year of professional experience as a registered/certified dietitian required.  Strong computer and organizational skills required.

The WIC Program Director is responsible for overall administration of the WIC Program. Responsible for determination of eligibility of applicants applying for the WIC Supplemental Nutrition Program in accordance with the Federal and State guidelines. Responsible for development of and implementation of a Nutrition Education program for WIC participants and WIC staff. Responsible for yearly State WIC required Nutrition Services Plan. Requirements:
PROOF OF REGISTERED DIETITIAN CREDENTIAL IS REQUIRED TO BE SUBMITTED WITH APPLICATION.

Registered Dietitian (R.D.) credentialed by the Commission on Dietetic Registration of the American Dietetic Association required. Administrative/Managerial Skills Essential. Three (3) years supervisory experience in WIC preferred. Exhibited sensitivity to cultural differences of clientele and demonstrated ability to work with public essential.

Clinical Nutrition Manager opportunity available in a progressive Northern North Dakota hospital

Position requirements:

  • Masters degree in nutrition or closely related field.
